Il giornalista Raoul Duke e il suo imponente avvocato, il dottor Gonzo decidono di partire a bordo di una decappottabile rosso scuro verso la città di Las Vegas. Nel bagaglio, sostanze farmaceutiche e bevande in grado di far uscire di testa chiunque.
"Fear and Loathing in Las Vegas" di Hunter S. Thompson nacque come reportage di una corsa in fuoristrada verso Las Vegas per la rivista "Sport Illustrated", ma si trasformò "on the road" in un viaggio nell'allucinazione e nell'America del sogno che stava cadendo a pezzi. Per più di 15 anni il libro rimbalza sulle scrivanie di registi e produttori hollywoodiani; ma è talmente estremo e dissennato che nessuno ha il coraggio di portare a compimento il progetto. Gilliam accetta il rischio. Certo non è un capolavoro come "Brazil", ma quasi un paradossale ritorno ai frammenti tragico-grotteschi che il regista componeva per il "Flying Circus".
